Concussion Baseline Test

In addition to completing all health forms, student-athletes must take a baseline ImPact test prior to participation. Student-athletes will receive specific instructions on how to complete the online test once their athletic registration is received.

Please note, student-athletes are required to take the baseline test every two years, so some student-athletes may not need to take this for the 25-26 school year. If you have any questions about the ImPact test during the summer, please contact our Athletic Office at athletics@pacificridge.org.

Sports Clearance Notification

Athletics will review and approve the Form ReLeaf athletic registration which includes the concussion baseline test results (if required).

An email stating that your student-athlete is cleared will come from Ms. Wiedemeier through Form ReLeaf. The email that was used to register the student-athlete will receive the clearance notification.

For Students

All Upper School students who plan on participating in a winter sport must complete the athletic interest survey. We will use the data collected to formulate initial rosters, communicate with interested student-athletes and families, and best prepare for the upcoming fall season.

Athletic Registration Clearance

Form ReLeaf is our Platform for all Upper School sport clearances. Student-athletes will NOT be able to participate in sport tryouts, practices, or competitions until they have gone through the clearance process AND received their clearance email from Ms. Wiedemeier through Form ReLeaf.
A submitted registration does not mean cleared for sports.
