Health and school registration forms
In early July, families received detailed instructions for uploading completed forms into our Magnus Health system. This platform will house all required health and registration documents for the 2025–2026 school year.

When visiting your healthcare provider, please bring the following forms as needed for your child’s specific health and school participation requirements:
Need Help Scheduling?
We understand that not all families can shift their physical schedule easily due to insurance restrictions. To support this transition, we’ve partnered with 828 Urgent Care Center in Oceanside (flyer and description of forms covered), which offers our families a discounted rate for one-time out-of-pocket annual physicals. This can help families “reset” their physical date to a summer month, easing future scheduling conflicts and aligning with school-year activities.
Please note: this option is not required, but we highly encourage you to consider it if it helps you align your child’s physical with a more optimal schedule.
Recap of required annual physical dates:
- Student-athletes must have an active physical on file at all times during their sport season.
- Global Travel Program participants must maintain a valid physical throughout any travel period.
- Our Health Office and the American Academy of Pediatrics recommends regular well-visits, including back-to-school physicals.
- Summer is an ideal time to take care of this important requirement before school and activities resume.
